The Social Worker works as part of your Rehabilitation Team and gives emotional support to you and your family during your Rehabilitation Programme at NRH, and will help you in solving personal problems that may arise.
Your Social Worker acts as a co-ordiniator of your care at during your admission period at NRH and also assists you with discharge planning for when you have completed your Inpatient Rehabilitation Programme.
Your Social Worker also provides you and your family with information you may need in the future about relevant services available in the Community.
