Description of Service
NRH Liaison specialists provide a link between the hospital, home, and other healthcare professionals involved in the person’s care.
They act as an advocate and liaise with referring hospitals to assess and evaluate the person’s readiness and medical stability for admission to an intensive Rehabilitation Programme.
As appropriate, the Liaison Service may continue to be available to advise and support patients and their family; and community healthcare professionals involved in their care.
