The Pharmacy at the NRH provides a comprehensive pharmacy service to all patients, carers and staff within all Rehabilitation Programmes at the NRH.
The Pharmacy Department has a key role in Interdisciplinary Team Working at the NRH.
Some examples of IDT working include:

- Pharmacy staff reconcile all medications on patients’ admission and discharge to ensure that the medication list is accurate at transitions of care.
- The goal of safe, effective, economic use of medicines is achieved through a close working relationship with medical and nursing staff.
- Reviews of all prescriptions are undertaken by Pharmacy to optimise medication therapy
- Pharmacists attend Consultant ward rounds to advise proactively on medications at point of prescribing. This also enables the pharmacy staff to engage with patients.
- Pharmacy staff participate in a weekly anti-microbial stewardship round with the Consultant Microbiologist
- Pharmacy dispenses medication for patients for therapeutic weekend leave, which is a vital element of patients’ rehabilitation.
- Individualised patient medications are dispensed – a safety initiative to decrease the risk of incorrect selection of medications on the ward
- Pharmacy liaises with community pharmacies and other hospitals regarding unusual, ’high tech’ and unlicensed medication issues that may arise
- Pharmacy presents an interactive workshop for all new doctors to advise on safe prescribing and supports nurse prescribers
- Pharmacists counsel patients on their medications prior to discharge on request and provide an individual medication list to each patient
- The Pharmacy staff at the NRH are actively involved in developing the service through continuing education and service initiatives

The Pharmacy service at the NRH provides a full pharmacy service for patients from each of the hospital’s five rehabilitation programmes (the Brain Injury; Spinal Cord Injury; Stroke Specialty; Prosthetic and Orthotic Rehabilitation and Paediatric Rehabilitation Programmes).
The Pharmacist on your Rehabilitation Team will:

- Work as part of the Interdisciplinary Team throughout your rehabilitation programme.
- Check your medications against your prescriptions on admission and discharge.
- Attend Consultant rounds to advise you and your rehabilitation team staff on medications at the point of prescribing.
- Help you to understand your medications and how to take them.
- Offers individual medication counselling and provides an individual medicines list
